The Community – Crownpoint, NM
Crownpoint is on the Trails of the Ancients Byway, one of the designated New Mexico Scenic Byways to prehistoric archaeological and geological sites of northwestern New Mexico. It provides insight into the lives of the Ancient Puebloans, Navajos, Utes, and Apache people. Geological features include canyons, volcanic rock features, and sandstone buttes. Several of the sites are scenic and wilderness areas with recreational opportunities. In 1912, Crownpoint was founded by Samuel F. Stacher as an Indian Agency to serve the Navajo People in the Pueblo Bonito Agency of Northwestern New Mexico. A school house, agency office, and power house were first built to accommodate future planned establishments.

Airport Information: Albuquerque International Sunport
The Albuquerque International Sunport, known for its distinct southwestern architecture and cultural décor, is New Mexico's largest commercial airport, welcoming over 5 million passengers each year. About the Facility – Crownpoint Health Care Facility
The Crownpoint Health Care Facility serves a population of 20,000 Navajo people. The staff consists of primary care physicians, physician assistants, and nurse practitioners at a 32-bed hospital. The daily inpatient load is nine; daily outpatient visits are 151; annual OB cases are 275; and annual ER cases number about 5,500.

Pharmacist - RESPONSIBILITIES (listed are included but not limited to)
- The Pharmacist shall navigate and utilize EHR systems for managing medical records within Indian Health Services.
- Process prescriptions accurately in adherence to medical guidelines.
- Verify prescription completeness, appropriateness, and interactions as per policies.
- Provide medication instructions and address medical queries following protocols.
- Manage controlled medications and document patient medical information according to guidelines.
- Engage in the Patient-Centered Medical Home (PCMH) model for patient care.
- Conduct specific medical orientations for new pharmacists on Client practices and policies.
- The Pharmacist maintains accurate medical records within established timelines.
- Participate in ongoing medical quality improvement programs aligned with Client protocols and standards.
- Screen patient charts to align medication orders with chart notes, lab work, vitals, and allergies.
- Process new medication orders and fulfill prescription orders as required.
- Ensure accuracy of filled prescription orders.
- Educate patients on medication therapy and provide necessary information.
- Collaborate with the pharmacy team and providers for timely and accurate pharmaceutical services.
- Address provider inquiries regarding drug therapy and ensure proper medication dispensation.
- Recognize and respond to life-threatening emergencies and adverse reactions to medications.
- Handle telephone calls, refill requests, and input outside prescriptions into the system.
- Conduct controlled substance audits as directed.
- The Pharmacist assists with order organization and performs patient pill counts at the pharmacy window when necessary.
Pharmacist - REQUIREMENTS
- Must have a degree from a school of pharmacy approved by a state-accrediting agency.
- Shall possess a current, active, unrestricted license from any of the 50 United States, the Commonwealth of Puerto Rico, or the District of Columbia.
- Shall maintain continuing education/certification and have a current Basic Life Support (BLS) certification.
- Must have a minimum of 2 years of experience as a pharmacist within the past 36 months.
- Shall meet all background investigation requirements.
